Emergency services were called to the crash scene, approximately 10km from the intersection of Braidwood and Albatross roads just before 8am.
A four-wheel drive travelling west and a truck and trailer travelling east collided.
The driver of the four-wheel drive died at the scene.
The truck continued on after the accident for around 50 metres, running off the road, before rolling and jackknifing near roadside bush.
Ambulance Rescue paramedics had to cut the driver, a man believed to be in his 60s, from the wreckage.
He was stabilised at the scene by paramedics and then airlifted by the NSW Ambulance Toll Rescue Helicopter from Albion Park to St George Hospital.
According to ambulance paramedics he suffered lower back, chest and pelvic injuries.
Braidwood Road remains closed in both directions as police investigate the crash.
The road is closed from Wugan Street, at Yerriyong, near the Albatross Aviation Technology Park and at Turpentine Road.
Motorists are advised to avoid the area, with diversions in place around the crash scene.
