About 10.30am on Wednesday a silver Holden ute travelling west on Forest Road and a silver Holden Captiva travelling east, collided about 1.5km west of the Callala Beach turn off.
The female driver of the ute suffered serious injuries and was being stabilised at the scene by Ambulance NSW paramedics.
A rescue helicopter from Albion Park was expected to land at the Callala Sports Field to airlift the injured woman to a Sydney hospital.
A family travelling in the Captiva, that was towing a trailer, were also treated by paramedics.
The male driver suffered bruising to the arms, neck and shoulders, while his wife and two children escaped with a few scratches.
The road will remain closed for a number of hours as crime scene officers investigate the cause of the accident.
