According to NSW Ambulance Illawarra district Inspector Terry Morrow, the 16-year-old male accidentally discharged the fishing speargun.
The incident occurred at 3.15pm on Monday, April 22, at Holiday Haven Bendalong.
"It's embedded in his lower leg, through his thigh," Insp Morrow said. "The spear has gone right through the muscle."
Insp Morrow said assistance was requested from Bomaderry Ambulance officials, who attended the scene.
He estimated the spear was about 90cm in length.
The teenager was transported to Shoalhaven District Hospital in a stable, "satisfactory" condition, according to Insp Morrow.
"There was not much bleeding at all - it went straight through, and the spear was left in situ so that it wouldn't cause any further bleeding," Insp Morrow said.
