The 17-year-old boy was taken to Dubbo Hospital just before 6pm Saturday to be treated for a wrist injury.
While the teen was being treated, he escaped custody and fled the hospital.
Police were alerted and officers from Orana Local Area Command conducted a large search for him but he was not located.
The juvenile is described as being of Aboriginal/Torres Strait Islander appearance, between 180cm and 185cm tall, with a thin build, black hair and brown eyes.
Police say he could be heading towards Orange, Bourke or Sydney.
Anyone who sees him is urged not to approach him but to contact triple-0 immediately.
