At 11am two women were stopped in the southbound lane on the highway, outside Emersons on Main, waiting to turn right.
A car being driven by a 91-year-old Burrill Lake woman then allegedly collided with the rear of the stationary vehicle causing both cars to travel forward more than 50 metres.
The drivers of both vehicles were injured in the crash, while the passenger of the stationary car was not hurt.
The 91-year-old was airlifted to St George Hospital, Kogarah for treatment for broken bones including a compound fracture to her right ankle.
Police report the woman remained lucid and in good spirits during the ordeal.
The Toll NSW Air Ambulance Rescue Helicopter landed on the grassed area of the Kings Point Road turnoff to collect the woman.
The driver of the stationary car, a 60-year-old Mollymook woman, was taken by ambulance to Shoalhaven Memorial District Hospital with a suspected spinal injury.
She complained of worsening numbness to her right side and neck pain.
Ulladulla Police, Ambulance, Fire and Rescue, and Roads and Maritime Services units attended the crash. The highway was closed to traffic in both directions for approximately one hour.
All vehicles were towed from the scene.
Police have spoken to a number of witnesses and have CCTV footage of the incident.
Police investigations are continuing.
