Two cars collided, and burst into flames 400 metres north of the Bendalong turnoff at 10.45am on Tuesday.
Three people died at the scene. Two women were air-lifted to hospitals in St George and Liverpool in a critical condition.
One lane of the Princes Highway was closed for most of the day.
Traffic remained heavy with southbound motorists facing 85 minute delays and northbound drivers facing 55 minute delays.
The Transport Management Centre urged drivers to delay their trip or take an alternative route.
Paramedics treated two women for multiple serious injuries including possible head injuries, an ambulance spokesman said.
An alternative route is available: northbound traffic can travel via the Kings Highway and Braidwood-Nowra Road and southbound traffic to take the Braidwood-Nowra Road and then Kings Highway.
